Transitions Coordinator – Based at Beaumont Lancaster with travel to Beaumont Blackpool. Part time  £17,958.70 (£29,931 FTE) Per annum. Term Time Only.

Job Overview 

Graduates of Beaumont College are extraordinary young people. Their College Curriculum alongside a wide range of highly personalised interactions and engagements with health, care, behaviour and technologists is intended to support them to fulfil their potential.

Our students enjoy their studies, have great relationships with the staff team and they commonly make progress that others didn’t believe was possible.

The Transitions Coordinator works directly with the students, their education, care, behaviour, technology and health teams, as well as parents/carers to look forwards to planning for, and engaging in busy and fulfilling lives beyond college. This can include further study, supported living, day services, voluntary or paid employment and of course, as much independence as possible whether that is through self-advocacy or applying for the benefits they may be entitled to.

Transitions Coordinators will attend Annual Review Meetings; maintain a current and up to date list of destination options. They will be excellent communicators and form strong relationships with all stakeholders to ensure the best possible, sustainable outcomes for our graduates.

Maintaining contact with our graduates is another key focus. Having an annual call for 3-5 years after graduate will; help the college to understand what was well planned, and what to do differently going forwards.

The post holder will work closely as part of each student’s multi-disciplinary team to be ambitious for the destinations of graduates.

Our students vary from following a sensory pathway, to being work ready, whether that be as a volunteer or in paid employment. Research tells us that young people with special educational needs and disabilities maintain their skills and thrive if they can continue to apply what they have learned. Securing high quality, busy and fun filled activities will be highly personalised. The Transitions Coordinator will have the databased, knowledge and understanding of services and relationships to promote positive destination planning.

Job Responsibilities – see full job description, to include: 

  • Maintain currency with new entrants and existing students to ensure their ambitions, goals and intended outcomes remain at the forefront of planning
  • Ensure planning is timely, taking into account the standard duration of study, and expected end date
  • Work with students and develop a mutually trusting relationship working within the boundaries of confidentiality the student has agreed to.
  • Understand and apply GDPR in data management, oversight and sharing; whilst working with the student to support a well-planned, positive destination transition over time via their education team.
  • Develop professionally appropriate relationships with parents and carers; ensure wider health and care stakeholder information is current and up to date
  • Work with health care professionals to ensure children transfer to adult services in health and care
  • Maintain accurate records and supporting evidence of the destinations of leavers for 3-5 year. Ensure timely reporting of destination data to MIS.
  • Develop a database of possible destinations taking into account student needs.
  • Ensure effective working relationships with all college staff, and be aware of key named staff for each student.
  • Use agreed record keeping systems effectively to record and evidence options, planning and decision making
  • Maintain currency with the Government Benefits System and support students through study and as they prepare to graduate to apply for funding to promote and support their independence as you adults
  • Ensure impartial, effective careers education, information and guidance is embedded in the delivery of learners for all students
  • Work with Cumbria and Lancashire Careers Hub with the assigned Careers Advisor
  • Use the Gatsby Benchmark statements to develop and embed the support arrangements essential for successful destination planning.
  • Work within all policies, procedures and guidelines
  • Any other duties commensurate with the post as determined by the Principal or their representative.
