Are you a qualified Social Worker looking for a varied and rewarding role where you can make a real difference to adults, families and carers within the community?
Jewish Care is looking for an experienced Social Worker to join our Social Work and Community Support (SWCS) service. You will provide professional social work support to adults across North East and Central London, Essex, supporting people to maintain their independence, improve their wellbeing and remain safely within their communities.
About the role
You will manage a complex caseload of approximately 35 cases (FTE), carrying out holistic assessments, developing person-centred support plans and delivering effective social work interventions. You will work with people experiencing a range of challenges, including ageing, disability, mental health needs, dementia, social isolation, end-of-life care and other complex or co-existing needs.
You will use your professional judgement to assess risk, respond to referrals, identify safeguarding concerns and ensure people receive the right support at the right time. The role also involves providing advice, information and advocacy, helping individuals and carers navigate health and social care systems and access appropriate services.
Partnership working will be an important part of the role. You will work closely with colleagues across Jewish Care and build effective relationships with statutory, voluntary and community organisations. You will also represent Jewish Care at external meetings and forums and contribute to the development of community-based initiatives, support groups and other activities that promote social inclusion and wellbeing.
About you
You will be a qualified and Social Work England registered Social Worker with experience of working with adults in a social care setting. You will have sound knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding, together with experience of assessment, care planning, risk management and social work interventions.
We are looking for someone who is confident working independently, while also contributing effectively to a multidisciplinary team. You will have excellent communication, listening, writing, organisation and IT skills, alongside the ability to exercise professional judgement and critically reflect on your practice.
Experience of community or group work is essential. Experience of working with people affected by mental health, dementia, disability, isolation, end-of-life care or other complex needs is particularly relevant. Experience within the voluntary sector or mental health services would be desirable.
Knowledge of the Jewish community would be advantageous, although a willingness to learn about Jewish life and develop your understanding of the community is essential.
Driving requirement
Although hybrid working is available, due to the geographical coverage of this role, you must hold a full UK driving licence and have regular access to your own vehicle for work purposes. The role involves travel across North East London, Central London, Essex, including visits within the community and attendance at meetings and services across different locations.
