Are you ready to be part of a service that’s transforming lives within the criminal justice system? At The Forward Trust, we are committed to supporting individuals in their recovery from substance misuse and mental health challenges. We’re currently recruiting a Health & Wellbeing Practitioner to join our dedicated team at HMP Coldingley, a closed Category C male prison in Surrey.
As part of an integrated healthcare framework, you’ll work alongside a multi-disciplinary team to deliver a mix of clinical and psychosocial interventions to adult male prisoners. This includes individuals presenting with low-level mental health needs and/or substance misuse issues.
You will be responsible for:
- Managing a caseload and delivering tailored recovery-focused interventions
- Conducting assessments and co-producing SMART care plans with service users
- Facilitating structured 1:1 sessions and group programmes
- Providing harm reduction advice and practical recovery support
- Supporting release planning and ensuring through-care into community services
- Performing second signatory duties as needed
- Maintaining accurate, up-to-date case records in line with service KPIs
This is a 7-day-a-week service, so weekend working will be required on a rota basis.

- Thrives in fast-paced, changing environments, with the ability to respond flexibly to service demands.
