Band 6 – Substance Misuse Community Nurse – Medway

Permanent

£37,338 – £44,962 DOE

We’re looking for a Full Time (35 Hours) registered Community Nurse to join our multidisciplinary substance misuse team, delivering high-quality, evidence-based care to people affected by drug and alcohol dependency across East Kent.

This community-based role offers the chance to make a real difference through assessment, treatment, harm reduction and recovery-focused interventions, always keeping the person at the centre of care.

What you’ll be doing

  • Deliver and supervise high standards of nursing care for people with substance dependency
  • Undertake triage, health assessments and ongoing clinical reviews alongside case managers
  • Provide 1:1 interventions, group work and brief interventions within a recovery framework
  • Support physical and mental wellbeing, including access to primary care and mental health services
  • Facilitate and support community alcohol detox programmes
  • Provide specialist clinical interventions, including health checks, ECGs, FibroScans, Pabrinex and Buvidal administration, as well as Hepatitis B vaccinations and Hepatitis C testing.
  • Promote harm reduction, including needle exchange, naloxone and OST awareness
  • Work closely with partner agencies to ensure effective through-care and safe discharge

What we’re looking for

  • Registered Nurse with current, active NMC registration (essential)
  • Experience or strong interest in community, substance misuse or mental health nursing
  • Confident in assessment, risk management and safeguarding
  • Skilled in motivational interviewing and person-centred communication
  • Able to work autonomously while contributing to a multidisciplinary team
  • Committed to evidence-based practice and inclusive, person-centred care
