Are you a skilled and compassionate nurse with a passion for supporting people affected by drug and alcohol dependency? The Forward Trust is looking for a Part-Time (21 hours, Monday – Wednesday) Band 5/6 Community Nurse to join our dedicated multi-disciplinary team in East Kent.
In this vital role, you will deliver and oversee high-quality nursing care for individuals experiencing substance dependency, supporting their health, recovery, and wellbeing.
You will play a central part in care planning, triage, brief interventions, detox support, and physical health monitoring, working closely with case managers and external agencies to ensure a coordinated, person-centred approach.
Key Responsibilities:
- Provide clinical interventions including health checks, ECGs, Buvidal, Hep C testing, and Pabrinex administration.
- Support the community alcohol detox pathway, ensuring safe monitoring and evidence-based care.
- Conduct health needs assessments, risk assessments, and contribute to care planning and delivery.
You will need to be…
- A qualified Registered Nurse (NMC registered) with experience working in substance misuse, community nursing, or a similar setting.
- Confident in working with clients with complex physical and mental health needs.
- Knowledge of OST, harm reduction, and detox protocols.
- Skilled communicator with excellent documentation and IT abilities.
- Compassionate, proactive, and collaborative. Someone who thrives in a team and is committed to inclusive, trauma-informed care.
